Nurse Navigator

Role Overview: The Nurse Navigator provides care coordination services to patients, families, and post-acute care providers, focusing on acute and chronic conditions, including women's health and pregnancy. Key Responsibilities: Facilitates interdisciplinary collaboration, coaches patients and caregivers in care planning and self-management, assesses understanding of disease, and reinforces discharge instructions. Collaborates with internal and external care teams, including physicians and managed care partners, to ensure seamless service delivery and optimal outcomes. Maintains comprehensive patient records, evaluates care processes, and addresses care issues. Additional Duties: Coordinates obstetric care, schedules consults and imaging, manages referrals and documentation, and supports educational initiatives. Works within the Fetal Care Center, training and cross-covering with other nurse navigators. Qualifications & Skills: Requires a Bachelor’s degree in Nursing or related field, valid RN license in Michigan, with at least two years of clinical and care coordination experience. Experience with process improvement methodologies like Lean is preferred.
