PACE Participant Onboarding Advocate
Habitat Health
Habitat Health is seeking a New Participant Advocate in California to welcome new participants into the PACE program. You will build relationships with participants and caregivers, coordinate onboarding, and liaise with the Interdisciplinary Team to ensure a smooth transition into care. The role emphasizes participant-centered support, education about services, and proactive communication to enhance the onboarding experience.
