Schedule: Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 - $100,000 per year plus $4,000 Sign-On Bonus

What You'll Do
•Provide skilled nursing visits in patient homes and care settings.
•Deliver hands-on care including wound care, medication management, and assessments .
•Build consistent relationships with patients and families.
•Coordinate care with physicians, therapists, and interdisciplinary teams.
•Educate patients and caregivers on care plans and recovery goals.
•Complete accurate, timely documentation .

What We're Looking For
•Active Minnesota Registered Nurse (RN) license.
•Experience in home health, med-surg, rehab, or clinical nursing .
•Ability to work independently while staying connected to a team.
•Strong communication and patient education skills.
•Reliable transportation and willingness to travel locally.
