Key Responsibilities\n

• Conduct thorough, evidence-based assessments to gain a deep understanding of each client’s needs and preferences.

• Develop, implement, and regularly update individualized treatment plans in collaboration with clients and healthcare team members.

• Provide individual and group therapy sessions via telehealth platforms, facilitating a safe and open environment for clients to express themselves.

• Maintain accurate and timely clinical documentation, including progress notes and care plans, adhering to organizational standards and compliance requirements.

• Engage with physicians, other healthcare providers, and support systems (family members and community resources) to ensure an integrated approach to treatment and stable client transitions.

• Assist clients with discharge planning, care coordination, and access to community services tailored to their specific recovery paths.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.

• Active state licensure as an LCSW, LMFT, LPC, or equivalent applicable credential.

• A minimum of 3 years of experience providing mental health counseling in an outpatient or similar setting.

• Proficiency in using various digital tools including electronic health records, video conferencing software, and basic office software such as Microsoft Word and Excel.

• Strong organizational skills and a self-directed work ethic are essential, as this role requires significant independence and personal initiative.

Preferred Experience\n
