Role Description The Physical Therapist will provide patient-centered care in an on-site setting located in West Palm Beach, FL, in a full-time capacity. Day-to-day responsibilities include conducting thorough evaluations, developing individualized treatment plans, and delivering evidence-based physical therapy interventions to improve mobility, strength, and functional independence. The role involves coordinating rehabilitation services, monitoring patient progress, and documenting clinical findings in accordance with professional and regulatory standards. The Physical Therapist will collaborate with caregivers and other service providers to support patients in home-like environments and during transitions, while promoting safety, comfort, and long-term wellness.
Qualifications
- Clinical expertise in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans.
- Strong foundation in Medicine and Patient Care, with a focus on safety, ethics, and evidence-based practice.
- Experience supporting patients in Home Care or community-based settings, including functional mobility and fall-prevention strategies.
- Current state license or eligibility for licensure as a Physical Therapist in Florida, and graduation from an accredited Physical Therapy program (DPT or equivalent).
- Effective communication and interpersonal skills, with the ability to work respectfully with diverse populations and multidisciplinary teams.
- Ability to manage a full-time caseload, prioritize tasks, and maintain accurate documentation in electronic or paper records. xhyhwjd
- CPR certification and familiarity with relevant regulations and best practices for therapy services in home and transitional care settings.
