Key Responsibilities of the RN - Home Health - Travel

• Perform comprehensive nursing assessments upon patient admission, considering all health factors.
• Develop data-driven care plans based on identified patient needs and case management objectives.
• Utilize HHRG values to guide visit frequencies and interdisciplinary resources appropriately.
• Refer patients to community resources as required.
• Participate in inter-disciplinary patient case conferences and evaluate care plan effectiveness.
• Provide supervision and documentation for Home Health Aide care plans bi-weekly.
• Communicate significant changes in patient conditions to relevant care providers.
• Complete all necessary paperwork and narrative documentation accurately and on time.
• Obtain and verify medical orders for all delivered care.
• Plan discharges appropriately and document all discharge procedures.

Requirements of the RN - Home Health - Travel

• Successful passage of VNA pre-hire medication test
• Current RN license in Massachusetts
• Minimum one (1) year of nursing experience, or six months with a BSN
• CPR certification
• Preferred: Home care and IV experience, BSN, computer skills
