In-Home CKD Care Manager RN Advocate
Monogram Health
Monogram Health is seeking a Registered Nurse Care Manager to improve patient lives by delivering in-home care, collaborating with nephrologists and a dedicated care team. You will manage a small patient panel, perform home visits, monitor vitals, coordinate medications, and educate patients and families on CKD, dialysis, and proactive care decisions.
