We are looking for a Social Services Assistant at Fulton Center for Nursing and Healing!

**Responsibilities:**  

- Develop and execute a comprehensive social services program, offering counseling and support services to residents and families 

- Assess and address residents' medical-related social needs 

- Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team 

- Advocate for residents' rights while liaising between all parties involved 

- Manage discharge planning efficiently 

- Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights

**Requirements:** 

- Bachelor’s Degree or higher, preferably in Social Work 

- Prior experience working in a long-term care setting

**Schedule:**

- Monday-Friday 8:00 AM to 4:30 PM
