Physical Therapist (PT)

We are seeking a compassionate and motivated Physical Therapist (PT) to join a growing Rehabilitation Team. The ideal candidate will be dedicated to helping residents regain strength, mobility, independence, and confidence while delivering high-quality rehabilitative care.

Roles & Responsibilities:

• Evaluate residents and develop individualized treatment plans.
• Help residents improve mobility, balance, strength, and overall functional abilities.
• Utilize therapeutic exercises, manual therapy techniques, stretching, and modalities to reduce pain and restore movement.
• Educate residents and their families throughout the rehabilitation process.
• Monitor resident progress and modify treatment plans as needed.
• Assist residents with the proper use of rehabilitation equipment.
• Supervise Physical Therapy Assistants as required.
• Collaborate with nursing staff, therapy teams, and other interdisciplinary healthcare professionals.
• Maintain accurate documentation and ensure compliance with all regulatory standards.
• Support a safe, clean, and resident-centered care environment.

Education, Licensure, and Experience:

• Graduate of an accredited Physical Therapy program.
• Current New York State Physical Therapist License required.
• Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) experience preferred.
• Strong communication, interpersonal, and teamwork skills.
• Passion for helping residents achieve their highest level of independence and quality of life.
