As a Hospice RN Case Manager, you'll play a vital role in planning, organizing, and directing hospice care in patients' homes and communities. You'll build on your nursing background and focus on community health education and support to meet the unique needs of individuals and families.

Key Responsibilities:
- Conduct comprehensive assessments of patients and families to determine hospice care needs.
- Develop, initiate, and revise care plans based on patient needs and status.
- Administer medications and treatments as prescribed by physicians.
- Offer counseling and support to patients and families, including education on self-care techniques.
- Coordinate patient care and communication with interdisciplinary teams.
- Participate in on-call duties and ensure necessary equipment and services are available.
Key Qualifications:
- Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and Active CPR certification.
- Valid driver's license, personal automobile, and insurance coverage.
- One year of hospice care experience is preferred.
- Case Management experience and supervision of hospice aides is a plus.
- Excellent observation, communication, problem-solving, and nursing skills.
- Ability to perform physical tasks such as lifting, positioning, and transferring patients.
- Effective social interaction, communication, and organizational skills.
