RN - Hospice

Overview This position will be traveling to see patients in the Portland Metro area (primarily West Portland). The Hospice Nurse under physician direction completes medical requirements while coordinating a whole person plan of care. Position is accountable for direct and indirect quality care for patients of all ages. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Position accepts professional development as an integral part of nursing practice. Coordinates patient care activities with other members of the health care team in a collegial relationship. Upholds the standards of confidentiality, attendance and punctuality. Responsibilities Travel to see patients in the Portland Metro area (primarily West Portland). Complete medical requirements under physician direction. Coordinate whole person plans of care according to the nursing process. Provide direct and indirect quality care for patients of all ages. Coordinate patient care activities with other health‑care team members in a collegial relationship. Uphold confidentiality, attendance, and punctuality standards. Qualifications Education to meet certification, license or registration requirement. Oregon Registered Nurse License (upon hire). National Provider BLS – American Heart Association (upon hire). 2 years medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, long‑term care or home health or hospice experience. Pay Range: $51.72 - $72.14. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
