Enrollment Case Manager

The Enrollment Case Manager (ECM) is responsible for facilitating the eligibility determination and enrollment process for Home and Community-Based Services (HCBS) waiver applicants. This role involves conducting in-home intake visits, assisting applicants throughout the application process, and collaborating with community organizations and healthcare providers to ensure effective service delivery. The ECM works independently within assigned counties and occasionally neighboring areas, maintaining cultural sensitivity and consumer-centered support. Responsibilities:

• Travel throughout assigned and neighboring counties to conduct in-home visits with applicants.
• Conduct intake visits for HCBS waiver applicants and facilitate the eligibility determination process.
• Present and discuss HCBS program concepts, advising applicants on their rights and responsibilities.
• Assist applicants with application submission and data entry into the Compass Application system.
• Support applicants in selecting Managed Care Organizations (MCOs), Life Providers, or Support Coordinators while maintaining consumer control.
• Develop and maintain relationships with community organizations, advocates, stakeholders, physicians, providers, nursing homes, and rehabilitation facilities.
• Identify and address stalled application cases to improve process efficiency.
• Attend scheduled meetings and report schedule availability regularly.
• Perform occasional lifting of items up to 25 pounds.
• Perform other duties as assigned by management.

• Bachelor's degree or equivalent combination of education and experience in social work, sociology, social welfare, psychology, gerontology, or related social science.
• Minimum of one year experience in public or private social work, or two years case work experience including one year performing functional ability assessments.
• Excellent organizational, written, and verbal communication skills.
• Ability to interact courteously and effectively with diverse audiences, including the disability community.
• Ability to work independently and handle multiple tasks under pressure.
• Experience entering data into automated information systems.
• Knowledge of local healthcare networks and providers.
• Cultural sensitivity and consumer-focused service approach.
• Valid driver's license and access to a personal vehicle (public transportation not allowed).

• Experience working in a local Area Agency on Aging (AAA) office.
• Familiarity with HCBS waiver programs and managed care systems.
• Established relationships with community-based organizations and healthcare providers.

• Must be willing and able to travel extensively within assigned and neighboring counties.
• Work primarily conducted in applicants' residences.
• Must have reliable personal transportation; public transit is not permitted.
• Occasional lifting of items up to 25 pounds required.

• Standard work schedule with regular meetings and reporting intervals.
• Flexibility to travel and conduct visits as needed based on business demands.
