RN Case Manager - Home Health

RN Case Manager responsible for providing coordinated primary care to 25-35 homebound patients, addressing physical, emotional, developmental, psychosocial, and educational needs. Responsibilities Assess, develop, and manage patient care plans, supervise LPNs and Home Health Aides, ensure effective communication with the care team, and participate in process improvements. Provide patient and caregiver education to promote self-care and independence. Maintain compliance with regulatory guidelines, complete documentation accurately, and participate in performance improvement activities.
