Dialysis Social Worker - Entry-Level, Impactful Care
Fresenius Medical Care
Fresenius Medical Care seeks an entry level MSW to provide psychosocial services to patients in our facility, including in-center and home dialysis patients when applicable. You will collaborate with the healthcare team to promote adjustment, rehabilitation and quality of life, educating staff on emotional and behavioral impacts of CKD. You will support patient education, counseling, and referrals, contributing to quality indicators and improvement activities while documenting all interactions #J-18808-Ljbffr
