Key Responsibilities

• Evaluate patients' physical abilities and develop comprehensive treatment plans tailored to prevent disability, relieve pain, enhance mobility, and restore optimal muscle function.
• Oversee the implementation of care plans by supervising Physical Therapy Assistants, ensuring treatments meet established guidelines and standards.
• Utilize advanced diagnostic techniques to assess muscle, nerve, and joint function, providing data-driven recommendations for ongoing care.
• Adapt rehabilitation strategies based on evaluations of patients' home environments and provide recommendations for modifications that promote independence.
• Deliver direct physical therapy services to clients as outlined in their physician-prescribed plans of care.
• Administer therapeutic exercises, traction, and massage to enhance physical recovery while educating clients on non-manual exercises.
• Evaluate, fit, and make adjustments to orthotic and prosthetic devices while collaborating with physicians for optimal patient outcomes.
• Document and maintain accurate clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with regulatory standards such as OASIS.
• Actively participate in interdisciplinary team meetings, contributing to case discussions, treatment planning, and progress evaluations.
• Engage closely with clients and their families, providing education on therapy programs and ensuring understanding of care practices.
• Complete all clinical and administrative documentation in a timely manner, adhering to agency policies.
• Participate in ongoing professional development and training, staying informed on the latest evidence-based practices in physical therapy.

Patient Population / Client Focus

This position focuses on a diverse population of patients who require in-home rehabilitation, including those recovering from surgery, suffering from chronic pain, and elderly clients facing mobility challenges. The Physical Therapist will tailor interventions to each individual's unique medical history, physical challenges, and personal goals, ensuring a holistic approach to recovery.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A minimum of one year of experience as a Physical Therapist in an acute care or home care setting.
• A valid and current physical therapy license in the state of Florida, maintaining good standing.
• Current CPR certification.
• A valid driver's license and automobile insurance.
• Strong interpersonal and communication skills, with a passion for enhancing patient care experiences.
• Demonstrated ability to work independently and within a collaborative healthcare team.

Preferred Experience

• Experience in home health care, particularly working with geriatric patients or in complex cases.

Compensation and Benefits

The position offers a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience and qualifications.
