Licensed Practical Nurse (LPN)

Position Summary

The Licensed Practical Nurse (LPN) is responsible for providing high-quality nursing care to residents in accordance with physician orders, individualized care plans, facility policies and procedures, and applicable federal and state regulations. The LPN works collaboratively with the interdisciplinary care team to promote resident safety, comfort, dignity, and overall well-being.

Essential Duties and Responsibilities

• Provide direct nursing care to residents in accordance with established standards of practice and individualized plans of care.

• Administer medications and treatments as ordered by physicians and document appropriately in the resident's medical record.

• Monitor residents for changes in condition and promptly report significant findings to the appropriate physician, Registered Nurse, supervisor, and/or responsible party as required.

• Perform routine nursing assessments and accurately document resident status, interventions, and outcomes.

• Assist with the development, implementation, and evaluation of resident care plans.

• Supervise and provide direction to Certified Nursing Assistants (CNAs) and other assigned nursing personnel within the LPN's scope of practice.

• Ensure physician orders are accurately transcribed, implemented, and followed.

• Obtain and monitor vital signs, blood glucose readings, weights, and other clinical measurements as ordered.

• Perform wound care and other nursing treatments within scope of practice and facility policy.

• Respond appropriately to resident emergencies and changes in condition.

• Communicate effectively with residents, families, physicians, pharmacy providers, laboratory services, therapy staff, and other members of the interdisciplinary team.

• Participate in shift-to-shift nursing reports and ensure accurate communication of resident needs and changes in condition.

• Maintain accurate, timely, and complete nursing documentation.

• Follow infection prevention and control practices, including appropriate use of personal protective equipment.

• Ensure residents' rights, privacy, dignity, and confidentiality are maintained at all times.

• Assist with admissions, transfers, and discharges as assigned.

• Participate in quality assurance, performance improvement, and resident safety initiatives as requested.

• Follow facility policies regarding medication management, controlled substances, incident reporting, and emergency procedures.

• Maintain a safe and clean resident care environment.

• Attend required staff meetings, in-services, and continuing education programs.

• Perform other nursing duties as assigned and within the LPN's authorized scope of practice.

Qualifications

• Graduate of an approved practical nursing program.

• Current and active Licensed Practical Nurse (LPN) license in the State of Florida or other authorization permitting practice in Florida, as applicable.

• Current CPR/BLS certification as required by facility policy.

• Previous experience in a skilled nursing, long-term care, rehabilitation, or geriatric setting preferred.

• Knowledge of nursing practices, medication administration, infection prevention, and resident safety.

• Ability to recognize and appropriately respond to changes in resident condition.

• Strong communication, documentation, organizational, and interpersonal skills.

• Ability to work effectively as part of an interdisciplinary healthcare team.

• Must meet all applicable employment, background screening, health, and facility requirements.

Knowledge, Skills, and Abilities

• Demonstrates sound clinical judgment within the LPN scope of practice.

• Ability to prioritize resident needs and manage multiple responsibilities.

• Ability to provide leadership and direction to assigned nursing staff.

• Demonstrates compassion, professionalism, and respect when interacting with residents and families.

• Ability to maintain confidentiality and comply with HIPAA requirements.

• Ability to accurately utilize electronic medical record systems and other required documentation systems.

Physical Requirements

The employee must be able to perform the physical functions necessary to provide resident care, which may include standing and walking for extended periods, bending, reaching, pushing, pulling, and assisting with resident movement or transfers. Reasonable accommodations may be provided in accordance with applicable law.

The LPN is expected to provide nursing services in accordance with applicable Florida law, federal requirements governing skilled nursing facilities, professional standards of practice, facility policies and procedures, and the resident's individualized plan of care.
