Description

• Conduct evaluations of patients in inpatient or outpatient settings and tailor individualized treatment plans to meet their needs
• Deliver physical therapy interventions focused on enhancing mobility, strength, balance, and overall function
• Collaborate with fellow nursing staff, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families in understanding therapeutic objectives, discharge processes, and home exercise programs
• Maintain accurate documentation of evaluations, treatments, and progress in line with organizational and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Any specialized certifications (for example, in neurology or geriatrics) are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
• Aptitude to treat patients facing complex medical and mobility issues
• Excellent communication and teamwork capabilities
• Proficiency in utilizing digital charting and clinical documentation software
