Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.Other duties as assigned.QualificationsHolds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education.Has one year of social work experience in a health care setting.Meets state and federal requirements for this position.Possess a clear and active Licensed Clinical Social Worker license in the state of Virginia.Current CPR certification.Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.Amedisys is an equal opportunity employer.
