Hospice Care Planner (RN)

Compensation

Salary Range: $115,000 - $125,000 per year

Location: Manhattan, NY

Position Summary
As a Hospice Care Planner (RN), you will support a comprehensive Hospice and Palliative Care program providing services across inpatient and community settings. This role focuses on evaluating patients referred for home health services, developing individualized plans of care, and facilitating smooth transitions from hospital to home care. You will also serve as a key liaison, building strong relationships with physicians, social workers, case managers, and discharge planners to ensure coordinated, patient-centered care.

• Key Duties:

• Evaluate patients and establish appropriate plans of care for home health services
• Facilitate admissions and coordinate discharge transitions to home care programs
• Collaborate with physicians, social workers, case managers, and discharge planners
• Maintain strong relationships within the medical center to support care coordination
• Ensure patient and family needs are addressed through effective communication and planning
• Support business development and outreach efforts within the healthcare system

Requirements

• Education

• Required: Graduate of an accredited School of Nursing
• Preferred: Bachelor's degree

• Licenses & Certifications

• Required: Active New York State Registered Professional Nurse (RN) license

Skills:

• Strong clinical assessment and patient evaluation skills
• Excellent written and verbal communication abilities
• Advanced critical thinking and problem-solving skills
• Ability to work independently and manage care coordination effectively
