**Sign On Bonus Opportunities** Multiple Full-Time and Part-Time Shifts Available Hourly Rate: $19.18 - $24.94 Grade 14
Join our Mental Health Transitional Living Program and help individuals with serious mental illness build independence, stability, and everyday life skills. This is a hands-on role where you’ll support residents in a structured, community-based setting.Key Responsibilities
- Serve as Qualified Mental Health Staff (QMHS) in compliance with NYS OMH regulations
- Manage a caseload and develop Individualized Action Plans (IAPs)
- Provide daily living skills training (ADLs), coaching, and recovery-based support
- Monitor and document residents’ mental, emotional, and behavioral health
- Provide crisis intervention and trauma-informed support
- Oversee medication administration , documentation, and med counts
- Coordinate care with healthcare providers, social services, and community resources
- Facilitate groups and skill-building activities
- Maintain a safe, structured, home-like environment
- Complete accurate documentation and Medicaid billing records
- Bachelor’s degree in Human Services or related field
OR - Associate’s degree + 3 years experience in a mental health setting
- Experience working with individuals with serious mental illness (SMI); co-occurring SUD/DD preferred
- Strong communication and documentation skills
- Ability to remain calm and make sound decisions in crisis situations
- Team-oriented with the ability to work independently
- Compassionate, respectful, and non-judgmental approach
- Valid NYS Driver’s License with acceptable driving record
- Reliable transportation for client-related travel
- Ability to navigate stairs and respond to emergency situations
