  We're looking for a compassionate, organized, and enthusiastic Director of Case Management to join our team!

  What You'll Do

  As our Director of Case Management, you'll play an important role in helping residents and their families navigate daily life at The Gardens. You'll work closely with our nursing, administrative, and caregiving teams to ensure residents receive the support, attention, and services they need.

  Some of your responsibilities will include:

  • Getting to know our residents and families, building relationships, and helping them feel supported and connected.

  • Leading and supporting our Case Management team.

  • Working with residents, families, and staff to develop individualized service plans that promote independence and quality of life.

  • Helping residents adjust to their new home and addressing concerns or challenges along the way.

  • Serving as an advocate for residents and helping ensure their voices are heard.

  • Coordinating services, appointments, transportation, and community resources when needed.

  • Participating in resident care conferences and collaborating with our interdisciplinary team.

  • Supporting staff through guidance, education, and teamwork.

  • Helping maintain a safe, supportive environment while ensuring compliance with applicable regulations and documentation requirements.

  Education & Experience Requirements

  Candidates must meet the applicable New York State Department of Health qualifications for this position, including one of the following:

  • A Bachelor's or Master's degree from an accredited college or university with major work in human resources or service delivery, along with at least one year of full-time experience providing services to a dependent adult population.

  OR

  • An Associate degree from an accredited college or university with major work in human resources or service delivery, along with at least three years of full-time experience providing services to a dependent adult population.

  New York State Department of Health written approval is required.
