The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, bereavement services and community education, outreach and referrals. Responsible for the delivery of varied social work services to hospice patients and families and actively participate as a member of the hospice interdisciplinary team. We focus on maximizing patient recovery, comfort, and independence for the highest possible quality of life. From post-surgery and post-hospital rehabilitation to long-term care for a chronic illness, our highly skilled care teams provide compassionate attention and specialized care every step of the way to reduce hospital readmissions and achieve exceptional outcomes.
