RN - Case Management

Care ManagerThis is a 13-week contract position.Day Shift: 5x8-Hour (08:00 - 16:30). A variable 5-day workweek with department operations occurring 7 days per week. Days off will vary and are not guaranteed to be back-to-back.The Care Manager will be responsible for care coordination and discharge planning for patients. This role requires assessing patient needs, developing treatment plans, and participating in family meetings.
