Partnering with an integrated care team, you will coordinate individualized care plans, support families through complex decisions, and offer comfort with professionalism, empathy, and respect.If you're a compassionate Registered Nurse who values meaningful patient connections and autonomy in your practice, we'd love to meet you.QualificationsRegistered Nurse Case Manager Requirements & Responsibilities:Associates, Bachelors, or Masters of an accredited nursing program (RN)1+ years of recent acute care experience required; hospice/home health experience preferredComplete initial and ongoing comprehensive assessments to determine hospice needsDevelop individualized care plans with the patient and family, updating as needs evolveAdminister medications and treatments as prescribedEducate patients and caregivers on care techniques, medications, diet, and symptom managementRotate on-call duties and ensure seamless care transitionsMust have reliable transportation, valid driver's license, and proof of auto insuranceBenefits & Schedule:Schedule: 8:00am to 5:00pm, Full-time position with on-call expectationsHealth, dental, vision, and life insuranceCompany-paid short/long-term disability and basic life insurance401(k) with up to 4% company matchTuition reimbursement and professional development opportunitiesPaid time off and paid holidaysThe starting salary range for this role is $75,000-$101,000 annual base salary. An employee's actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
