Office Location: 20 Lake Street North, Ste. 201, Forest Lake, MN 55025
Coverage area: Washington / Anoka / Chisago Counties

Schedule: Mon-Fri, 8:00 AM - 5:00 PM

As a RN Case Manager You will: 

Assess/monitor physical, emotional, and psychological needs of patients

Create hospice care plans that align with the patient's wishes and goals

Direct nursing care: administering medications, treatments, and interventions

Provide pain and symptom management

Educate and support the patient’s family and caregivers

Collaborate with interdisciplinary team

Maintain accurate and timely documentation
