We are seeking a compassionate, organized, and proactive Home Care Scheduler / Care Coordinator to join our growing home care team. This key position is responsible for coordinating caregiver schedules, supporting client care needs, and assisting with caregiver recruitment, onboarding, and retention efforts. The ideal candidate thrives in a fast-paced environment, excels at relationship-building, and enjoys working with both clients and caregivers to ensure exceptional service delivery.

The successful candidate will serve as a primary point of contact for clients and caregivers, ensuring continuity of care, maintaining staffing coverage, and supporting Human Resources functions that contribute to a positive employee experience and agency growth.

 

Key Responsibilities

Scheduling & Care Coordination

• Manage and coordinate daily caregiver schedules to ensure all client shifts are staffed appropriately and efficiently.
• Match caregivers with clients based on care needs, skills, availability, location, and compatibility.
• Monitor schedules and proactively address call-offs, schedule changes, and urgent staffing needs.
• Coordinate service starts, schedule updates, and ongoing care needs with clients, families, caregivers, and healthcare professionals.
• Maintain accurate scheduling records using platforms such as WellSky Personal Care, AxisCare, Generations, or similar home care software .
• Review caregiver availability and workload to maximize coverage and service quality.
• Support client admissions, care plan updates, and continuity of care initiatives.
• Ensure all client and caregiver information is maintained accurately and in compliance with HIPAA and agency standards.

Recruiting, Human Resources & Onboarding

• Support caregiver recruitment efforts by screening applicants, scheduling interviews, conducting reference checks, and maintaining applicant records.
• Assist with onboarding new caregivers, including orientation scheduling, new hire paperwork, credential collection, and system setup.
• Track required employment documents, certifications, licenses, background screenings, and training requirements.
• Maintain employee records and ensure compliance with agency policies and regulatory requirements.
• Coordinate training, continuing education, and professional development opportunities for caregivers.
• Participate in caregiver engagement and retention initiatives to promote employee satisfaction and reduce turnover.
• Assist with payroll preparation, timesheet review, visit verification, and attendance tracking.
• Support human resources activities including employee communications, recognition programs, and performance documentation.
• Serve as a resource for caregivers regarding schedules, policies, benefits information, and onboarding questions.

Administrative Support

• Answer and direct incoming calls while providing exceptional customer service.
• Maintain organized filing systems, employee records, and client documentation.
• Assist with office operations, correspondence, document management, and administrative projects.
• Support team meetings, training sessions, recruiting events, community outreach activities, and other agency initiatives.

 

Qualifications

• Previous experience as a Home Care Scheduler, Staffing Coordinator, Care Coordinator, Recruiting Coordinator, Human Resources Assistant, Client Services Coordinator, or related healthcare administrative role required.
• Experience in home care, home health, private duty, hospice, healthcare staffing, or a related healthcare setting strongly preferred.
• Experience with caregiver recruitment, onboarding, employee records management, or human resources processes preferred.
• Proficiency with WellSky Personal Care, AxisCare, Generations Homecare System, ClearCare, or similar scheduling software strongly preferred.
• Demonstrated experience coordinating schedules and managing staffing challenges in a fast-paced environment.
• Strong understanding of caregiver-client matching and service coordination processes.
• Excellent organizational, multitasking, and problem-solving skills.
• Ability to effectively manage competing priorities and respond to urgent staffing needs.
• Exceptional communication and interpersonal skills with the ability to build positive relationships with clients, caregivers, and team members.
• Knowledge of HIPAA requirements, employment documentation, and healthcare compliance standards preferred.
• Proficient in Microsoft Office Suite and general office technology.
• Experience with payroll support, timekeeping systems, and employee onboarding platforms preferred.
