Nurse Practitioner (Maternity Leave - 2 positions)

Responsibilities Provide primary health care services to patients of all ages in a Community Health Centre (CHC) model of care, including health assessments and diagnosis, health care management and therapeutic interventions, screening, referrals, education, follow‑up, as well as health promotion and prevention of illness and injury in accordance with the Standards of Practice for Nurse Practitioners. Collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate patient care in consultation with patients and their families. Participate in the development, delivery, and evaluation of educational and health promotion programs within the community as needed. Qualifications Bachelor of Science in Nursing. Registered Nurse in the Extended Class (as recognized by the CNO). Masters in Nursing preferred. Preference for experience in a community health or primary care setting. CPR and First Aid certification. Knowledge and experience in health promotion, education and program evaluation. Knowledge of community resources, especially in health and social services. Ability to work within an interdisciplinary team. Comprehension of French and English preferred. Proficient with computers, Microsoft Office, and digital communication tools.
