Outpatient Physical Therapist

Job Description

We are seeking a dedicated and skilled Outpatient Physical Therapist to join our team starting August 17. This position will involve working between two locations: Delmonico and Garden of the Gods. This is a full-time role with a focus on patient evaluations and follow-up treatment plans.

Schedule

This position involves a 5x8.0 schedule, with 40 hours per week.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop, implement, and modify treatment plans based on patient needs and physician directives.
• Delegate and direct therapy services, ensuring functional treatment programs are designed for effective community reintegration.
• Educate patients, families, students, and coworkers while adhering to professional standards and organizational values.
• Maintain accurate and timely medical record documentation compliant with state and federal guidelines.
• Engage in professional development and adhere to state practice acts along with continuing education requirements.
• Participate in student programs, providing supervision and constructive learning environments.
• Contribute to quality improvement initiatives and provide feedback to enhance service delivery.

Required Experience / Certifications / Licensure

• A degree in Physical Therapy from an accredited school is required.
• CPR certification is required.
• A valid state license to practice as a Physical Therapist is required.
• A minimum of 6 months of outpatient clinical experience is required.
