Position Details:
Patient Population: Geriatric
Setting: PACE / Outpatient Community-Based Program
Responsibilities:

Provide medical direction and supervision of participant care
Oversee clinical outcomes and QAPI program
Supervise PCPs and APPs
Conduct history and physicals, reassessments, and care planning
Participate in interdisciplinary team meetings and on-call rotation

Required Procedures: Not specified
EMR System: Not specified
Support Staff: PCPs, APPs, interdisciplinary care team
Days Per Month: Full-time
On-Call/Weekends: On-call rotation as required

Submission Requirements:

Active Massachusetts medical license
Board Certified in Internal Medicine or Family Practice
Board Certification in Geriatrics (Preferred)
CDS Certification and DEA Registration
Previous supervisory experience
Minimum one (1) year experience with frail or elderly population
Most recent NPDB Self Query (within 30 days)
