Position Summary The Director of Resident Care (RN) is responsible for providing clinical leadership, resident care oversight, and operational support for Fresno Guest Home communities. This position ensures the delivery of high-quality resident care through effective nursing leadership, regulatory compliance, staff development, and quality improvement initiatives. The Director of Resident Care collaborates with Executive Directors, Regional Leadership, physicians, nurse practitioners, ancillary providers, and interdisciplinary teams to promote positive resident outcomes while maintaining compliance with all federal, state, and local regulations. The Director of Resident Care serves as the resident care leader for Fresno Guest Home communities by overseeing caregivering, supervising clinical staff, developing clinical systems, supporting admissions, maintaining licensing readiness, and fostering a culture of excellence, accountability, and resident-centered care. Essential Duties and Responsibilities Nursing Leadership Provide leadership, direction, and oversight of caregiving services within Fresno Guest Home communities. Supervise and mentor caregivers, field supervisors, CNAs and other clinical Fresno Guest Home personnel. Promote professional care standards and evidence-based practices. Assist Executive Directors with resident care decision-making. Ensure care services are delivered in accordance with physician orders and company policies. Serve as the primary care resource for community leadership and staff. Care Operations Oversee resident assessments and evaluations, and development of individualized resident care needs. Review resident health status and intervene appropriately when changes in condition occur. Coordinate resident admissions, re-admissions, transfers, and discharges. Coordinate and Collaborate with physicians, nurse practitioners, pharmacies, hospice agencies, home health providers, therapy providers, and other healthcare professionals. Oversee medication management systems, including medication storage, documentation, audits, and error prevention. Ensure appropriate follow-up for incidents, falls, infections, wounds, hospitalizations, and significant changes in resident condition. Monitor infection prevention and control practices throughout each community. Regulatory Compliance Ensure compliance with California Department of Social Services (CDSS), California Department of Healthcare Services (DHCS), OSHA, HIPAA, and all applicable federal, state, and local regulations. Maintain continuous licensing readiness. Conduct routine clinical audits to ensure compliance with company standards. Assist with licensing surveys, complaint investigations, and Plans of Correction. Monitor documentation to ensure accuracy, completeness, and regulatory compliance. Quality Assurance & Performance Improvement Develop and oversee Quality Assurance and Performance Improvement (QAPI) activities. Analyze health indicators including falls, medication errors, infections, weight loss, skin integrity, hospitalizations, and resident outcomes. Identify trends and implement corrective actions to improve care performance. Monitor quality metrics and provide ongoing recommendations to leadership. Staff Development Mentor, coach, and evaluate care operations personnel. Participate in orientation and onboarding of new Field Supervisors. Ensure ongoing competency assessments and continuing education. Promote employee engagement, accountability, and retention through effective leadership and coaching. Resident & Family Services Participate in resident care conferences as needed. Communicate with residents and families regarding care concerns and changes in resident condition. Assist in resolving resident and family concerns in a timely and professional manner. Promote resident dignity, independence, and quality of life. Administrative Responsibilities Maintain accurate care records and reports. Participate in leadership meetings and interdisciplinary committees. Support company initiatives, policy implementation, and strategic objectives. Qualifications Required Current California Registered Nurse (RN) license in good standing. Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) required. Minimum five (5) years of progressive nursing experience. Minimum three (3) years of nursing leadership experience in assisted living, memory care, skilled nursing, home health, hospice, or other post-acute healthcare settings. Strong knowledge of geriatric nursing principles and chronic disease management. Working knowledge of California RCFE regulations and healthcare regulatory requirements. Excellent leadership, organizational, communication, and critical thinking skills. Valid California driver's license with reliable transportation and the ability to travel between assigned communities. Preferred Experience overseeing multiple senior living communities. Experience with state licensing surveys and regulatory compliance. Certification in Gerontological Nursing or Nursing Administration. Experience implementing quality improvement initiatives. Knowledge, Skills & Abilities Advanced clinical assessment and nursing judgment. Strong leadership and supervisory skills. Knowledge of medication management and infection prevention. Experience with quality assurance and regulatory compliance. Strong documentation, auditing, and analytical abilities. Excellent interpersonal and conflict-resolution skills. Ability to manage multiple priorities and work independently. Proficiency with electronic health records and Microsoft Office applications.
