Home Care Scheduler

The Home Care Scheduler is responsible for coordinating the scheduling and tracking of home care and home care assistant assignments. The Home Care Scheduler will provide outstanding customer service to all stakeholders, including participants, families, vendors, and staff. The Scheduler will maintain current and accurate records of all scheduled appointments. Collaborates with colleagues and stakeholders to promote company values, team, and culture. The Scheduler will effectively coordinate the scheduling of home care personal care assignments as directed by the Home Care Supervisor. This includes adjusting schedules based on changes and communicating with all stakeholders. The Scheduler must ensure required authorizations in the electronic medical record for approved personal care. They will also ensure all care logs and timesheets are received from vendors and in-home staff visits. Consistently implement procedures to ensure a successful experience for providers, staff, and participants. This may include reminder calls, instructions, or sending materials ahead of time. Maintain current and accurate records of all scheduled home care personal care assignments, including scheduled times and outcomes. The Scheduler must be willing and able to work a varied schedule that may include evenings, weekends, and overtime. Complete all required documentation in a timely and accurate manner. Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families. Follow company policies and procedures and participate in any required quality improvement activities, staff training, and meetings. Communicate regularly with the Supervisor and staff regarding workload and priorities. Timely completion of all mandated trainings and education is essential. Exercising flexibility in performing assignments as business needs evolve is expected. Additional duties may be assigned as needed.
