Physical Therapist – Skilled Nursing Facility Join a dedicated healthcare team as a Physical Therapist in a Skilled Nursing Facility (SNF) located in Pueblo, Colorado. Provide direct physical therapy interventions, including therapeutic exercises, manual therapy, gait training, balance activities, and the application of various modalities. Educate patients and their families on treatment plans, home exercise programs, and adaptive equipment use to promote continued progress and independence. Accurately document patient evaluations, treatment progress, and discharge summaries in compliance with facility and regulatory standards. Collaborate effectively with physicians, occupational therapists, speech‑language pathologists, nurses, and other healthcare professionals to ensure integrated patient care. Monitor patient progress, modify treatment plans as necessary, and prepare for discharge planning. Adhere to all facility policies, procedures, and ethical guidelines. Required Qualifications Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
