Sangre de Cristo Community Care is seeking a Full-Time Physical Therapist to join our growing Home Health team serving the Pueblo and La Junta communities. About the Role

As a Home Health Physical Therapist, you will work one-on-one with patients in their homes, helping them regain mobility, independence, and confidence. You will:

• Evaluate patients’ physical conditions and functional abilities
• Develop and implement individualized, evidence-based treatment plans
• Educate patients and caregivers to support long-term success
• Collaborate with an interdisciplinary team focused on exceptional patient outcomes

What We Offer

• Competitive Salary: $94,000–$112,000 annually
• Mileage Reimbursement: $0.625 per mile

Requirements:

• Must be at least 21 years of age
• Graduate of an accredited Physical Therapy program (APTA/CAHEP) with a B.S., B.A., or M.S. degree
• Licensed or eligible for licensure as a Physical Therapist in the State of Colorado
• Valid, non-restricted driver’s license and reliable transportation
• Ability to work a weekend schedule as needed
• Minimum of two (2) years of experience required; home health experience preferred
