Sangre de Cristo Community Care is seeking a Full-Time Physical Therapist to join our growing Home Health team serving the Pueblo and La Junta communities. About the RoleAs a Home Health Physical Therapist, you will work one-on-one with patients in their homes, helping them regain mobility, independence, and confidence. You will:
- Evaluate patients’ physical conditions and functional abilities
- Develop and implement individualized, evidence-based treatment plans
- Educate patients and caregivers to support long-term success
- Collaborate with an interdisciplinary team focused on exceptional patient outcomes

What We Offer- Competitive Salary: $94,000–$112,000 annually
- Mileage Reimbursement: $0.625 per mile

Requirements:- Must be at least 21 years of age
- Graduate of an accredited Physical Therapy program (APTA/CAHEP) with a B.S., B.A., or M.S. degree
- Licensed or eligible for licensure as a Physical Therapist in the State of Colorado
- Valid, non-restricted driver’s license and reliable transportation
- Ability to work a weekend schedule as needed
- Minimum of two (2) years of experience required; home health experience preferred
