We are seeking an experienced and motivated Clinical Nurse Manager TO oversee daily clinical operations of Haven Of care home health agency while ensuring compliance with Indiana State regulations, Medicare Conditions of Participation, Medicaid requirements, and agency policies. This role provides leadership and supervision to clinical staff, coordinates patient care services, supports admissions and scheduling processes, and maintains survey readiness at all times.
The ideal candidate is a seasoned home health RN with leadership experience, strong knowledge of OASIS and Plan of Care requirements, and a commitment to person-centered care.
Key Responsibilities
Clinical Operations & Patient Care Management
– Oversee and coordinate all patient care episodes, including Start of Care (SOC), recertifications, resumptions of care, transfers, and discharges
– Provide direct oversight of OASIS documentation to ensure clinical accuracy, regulatory compliance, and accurate outcome measurement
– Supervise field Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Home Health Aides (HHAs) in the delivery of patient care services
– Ensure timely completion and accuracy of physician orders, Plans of Care (485s), verbal orders, and all care coordination documentation
– Monitor patient outcomes and intervene proactively to ensure appropriate skilled interventions are in place
– Collaborate with physicians, discharge planners, and case managers to ensure seamless transitions and continuity of care
Quality Assurance & Regulatory Compliance
– Lead agency quality assurance and performance improvement (QAPI) activities, including clinical chart audits and outcome analysis
– Ensure ongoing compliance with Medicare, Indiana Medicaid, state licensing regulations, and agency-specific policies and procedure
– Maintain continuous survey readiness and actively support preparation for state and accreditation surveys
– Monitor and enforce standards related to infection control, documentation timeliness, and patient safety
– Assist in the development and implementation of clinical policies, procedures, and staff education initiatives
Staff Leadership & Development
– Coordinate field clinician and caregiver scheduling in collaboration with office and administrative staff
– Conduct new employee clinical orientation, competency evaluations, supervisory visits, and ongoing in-service training
– Provide HHA supervision visits per Medicare Conditions of Participation and state requirements
– Facilitate case conferences and promote interdisciplinary team collaboration to optimize patient care planning
– Support staff coaching, performance improvement plans, and retention strategies
Medicaid & Medicare Documentation Support
– Support documentation requirements for Medicare and Indiana Medicaid home health services, ensuring medical necessity is clearly established and supported
– Assist with prior authorization processes, care coordination, and payer-specific compliance activities
– Ensure clinical documentation meets reimbursement standards and is completed within required timeframes
Qualifications & Requirements
Required
– Active, unrestricted Indiana Registered Nurse (RN) license in good standing
– Minimum of 2 years of clinical experience in a home health setting
– Demonstrated knowledge of OASIS documentation, Medicare Conditions of Participation, and Medicaid home health regulations
– Strong written and verbal communication skills with the ability to lead, mentor, and motivate staff
– Valid Indiana driver's license and reliable personal transportation
Preferred
– Prior experience in a Clinical Manager, Case Manager, or supervisory leadership role in home health
– Familiarity with Electronic Visit Verification (EVV) systems and home health software platforms
– Experience supporting state surveys, accreditation reviews, or QAPI program development
