Memory Care Coordinator

Responsibilities Facilitate the development, implementation and evaluation of program services and initiatives. Plan, implement, evaluate, and coordinate appropriate patient care. Responsible for determining appropriate trainings for the staff and developing the learning program. Collaborate with others to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures. What Minimum Requirements You’ll Need Licensure / Certification / Registration Advanced Life Support preferred. American Heart Association or American Red Cross accepted. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
