Nurse Practitioner, Care Management

Job Overview The Nurse Practitioner, Care Management provides care management and primary care medical services to high-risk patients. The NP manages the assigned patient panel. Minimum Requirements Nurse practitioner with an active, unrestricted California license to practice medicine. Minimum 2 years of experience and training as a nurse practitioner. At least 1 year of experience in a high-risk specialty such as Geriatric, Hospice, or Palliative Medicine. Valid BLS certification or higher, following the American Heart Association (AHA) or American Red Cross guidelines. Experience with computer-based systems; Electronic Medical Record experience preferred. Compensation Annual salary range: $138,257.60 – $172,806.40. Actual offers are based on budget, experience, skills, education, licensure, and certifications. The role may also qualify for discretionary bonuses or incentives.
