Description The Clinical Pharmacist has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Required Qualifications PharmD Pharmacy Or Bachelor's Degree Pharmacy Upon hire: Texas Pharmacist Registered Or Upon hire: New Mexico Registered Pharmacist - New Mexico Board of Pharmacy Preferred Qualifications Acute care hospital pharmacy experience, 1 year or equivalent experience Completion of ASHP-accredited PGY1 residency Why Join Providence?
