Contacts referring provider office for new order if incomplete.Manages work queue for nutrition and diabetes services for multiple locations throughout the systemFollows scheduling policy and procedures to accurately identify patient, insurances, referrals, and obtains required signatures where applicableAccurately screens ABNs to determine medical necessity, explains ABN to patient, obtains signatures and reviews document for completeness.Provides basic nutrition and diabetes specific health informationGenerates reports to identify cancellations/no shows and reviews service history to determine need for reschedulingFacilitates collection of data for CQI & KPI initiativesObtains appropriate referrals for outpatient follow-up services post-discharge from hospitalConnects with patients post-discharge from hospital to complete patient experience surveyCommunicates with referring providers for any patient not receiving services as prescribed (ie unable to contact, no-show, multiple cancellations, etc)Generates daily reports for inpatient glycemic dataAssists with data collection for annual accreditation reportingSchedules conference rooms for group session, staff meetings, trainings, etcAssists with management of staff outlook calendarsGeneral clerical duties as assignedCompensation (Commensurate with experience):$18.28To$27.42To access our Benefits Guide/Plan Information, please click the link below: #J-18808-Ljbffr Southern Illinois Healthcare
