Endocrinologist - Value-Based Care & Chronic Disease Leader
Upperline Health
Upperline Health is seeking a board-certified or eligible Endocrinologist (MD/DO) to join our multidisciplinary endocrine care team in a clinic-based setting, focusing on diabetes and endocrine disorders, and preventing complications such as foot ulcers. You will develop individualized care plans, order and interpret labs/imaging, educate patients on CGMs and insulin pumps, document in EMR, and participate in quality improvement and care planning. #J-18808-Ljbffr Upperline Health
