About This Role The RN Field Clinical Care Coordinator will play a crucial role in an Integrated Care Model by advocating for members' needs and ensuring they receive appropriate services. This position is ideal for those who are flexible, adaptable, and patient in various situations. Highlights Location: Boston, MA Schedule: Full Time, Day shift Work Style: Field-based Travel: Ability to travel in assigned regions to visit members in their homes and/or other settings What You'll Do Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment. Assess, plan, and implement individualized care strategies directed toward the most appropriate level of care. Identify and initiate referrals for social service programs, including financial and community services. Manage the care plan throughout the continuum of care as a single point of contact. Partner and collaborate with internal care teams, providers, and community resources to implement care plans. Communicate health-related information to ensure quality coordinated care and services.
