Registered Nurse RN Case Manager Home Health Honesdale, PA (Wayne County, Hawley, Carbondale Area) Interim HealthCare of Scranton $38–$40/hour + $5,000 Sign-On Bonus + Weekly Pay Immediate opening for a Registered Nurse RN Case Manager Home Health in Honesdale, PA and surrounding areas. SCHEDULE
Monday through Friday 8:30 AM to 5:00 PM On-call, weekend, and holiday rotationREGISTERED NURSE RN CASE MANAGER RESPONSIBILITIES
Manage a home health caseload in Honesdale and surrounding areas Perform RN assessments and reassessments Develop and update care plans Coordinate care with physicians and healthcare team Complete home health documentation and ensure compliance Educate patients and familiesREQUIREMENTS
Active Pennsylvania Registered Nurse RN license Minimum 1 year home health RN experience Case Manager or Case Management experience preferred CPR certification Valid driver’s license and reliable transportationABOUT INTERIM HEALTHCARE OF SCRANTON
