Shift: 8hr Day Shift - 40

Shift Information: 0800 - 1700

Clinical Interviewing- No - Manager Interview

On Call, Call Back Requirements:  Call is not required, but may be available if interested

Weekend Rotation:  None, unless willing and interested in working weekends

Holiday Requirements:  Travelers will work 75% of holidays

Floating Requirements:  Will travel throughout the following areas: Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and cities in between

About Facility:

• Radius rule: Traveler's permanent address must be 50 miles outside of home unit

Department Information:

• Teammates are required to complete 6 visits a day (routine visits, discharge, ROC, recert, reassessment)

Minimum Years of Experience required: 1 year Home Health experience

• Will you accept a first-time traveler: Yes, if seasoned to home health

Patient Types/Common Diagnoses:

• Chronic diseases, wound care, IV, pleurx, wound vac
• There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
• Teammates will travel throughout the below areas:
  • Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and cities in between

Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:

• Driver's License and proof of Insurance
• BLS

Skills required:

• Homecare Homebase experience is required (no exceptions), IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS, case management (ability to oversee/episode management up to 25-30 patients at once)

Support within the Department:

• Departmental leadership
• Schedular
• Clinical Manager
• Clinical Supervisor
• Peers/Clinical Team

Technology/Equipment:

• EPIC Rover – view only
• Focura - will upload discharge instructions and other important docs such as wound pictures
• Samsung note 9 phones
• EMR: HCHB (Homecare Homebase) – experience required

Orientation (facility and unit):

• NEO
• 5 days orientation

Scheduling:

• Weekend rotation: none, unless willing and interested in working weekends
• Call is not required, but may be available if interested
• Holiday Expectations: Travelers will work 75% of holidays
• RTO: Manager approval
• Schedule cycle: every 13 weeks
