Job Title: RN Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: 1 year Home Health experience required License: Driver's License and proof of Insurance Certifications: BLS Must-Have: Homecare Homebase experience required Description: Home Health nursing involves providing care to patients in their homes. Teammates are required to complete 6 visits a day, including routine visits, discharges, reassessments, and recertifications. There is no maximum mileage to drive, with an average of 60-80 miles a day. Teammates will travel to various locations within the designated area. Patient types include those with chronic diseases, wound care needs, and IV therapy. The required skills include IV therapy, wound care, assessment, and disease teaching. Experience with OASIS and case management is necessary, with the ability to manage up to 25-30 patients concurrently. A phone will be provided for documentation and communication with patients and providers. The department fosters a team environment and has strong support from departmental leadership. Weekend work is not required unless the individual expresses interest. Holidays will require work 75% of the time. Requests for time off will need manager approval. The culture within the department values teamwork and collaboration. Scheduled visits with patients will occur the day prior to the planned visit. Cancelled appointments will not count toward visit equivalents; a total of 30 visit equivalents per week is expected. No company car will be provided. This position requires strong time management and organizational skills.
