Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Job Description

We are seeking a dedicated Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) for a long-term care setting. This position requires a commitment to providing high-quality patient care while fostering a supportive environment for residents.

Responsibilities

• Continuously observe residents for changes in mental and physical well-being; communicate observations to the Supervisor and appropriately notify the physician.
• Receive, transcribe, and execute physician's orders accurately.
• Administer medications and treatments per physician's orders, ensuring proper documentation via computerized systems (MARs/TARs).
• Supervise Licensed Nursing Assistants (LNAs) in accordance with licensing requirements, ensuring care provided meets qualifications and scope of practice.
• Accompany the physician during resident rounds to address medical needs and facilitate communication during reviews.
• Administer oxygen and manage gastrostomy/jejunostomy feedings in accordance with facility policies.
• Perform routine duties, including managing appointments, checking therapy schedules, and overseeing documentation practices.
• Admit, discharge, and transfer residents, completing all necessary documentation and notifications.
• Compile and maintain the 24-hour report for assigned unit(s) and communicate consistently with management regarding resident issues.
• Communicate with residents' families to discuss concerns and obtain consent when necessary.
• Manage the assigned unit during shifts, ensuring compliance with state and federal regulations regarding medications.
• Assist with orienting new nursing employees and evaluate nursing staff according to facility policies.

Requirements

• Minimum of 3 years of current experience in long-term care.
• Must be willing to float to different units as needed based on experience.
• IV certification required (must be obtained during onboarding if not certified).
• COVID vaccination is mandatory; declinations or exemptions are not accepted.
• Active New Hampshire nursing license required at time of submission.
• Preferred: Previous travel experience and experience in rural/small facilities.

Working Conditions

This position operates primarily during day shifts from 7 AM to 3 PM, with weekend shifts required every other weekend. Candidates must have their own transportation, as the facility is located in a rural area with limited housing options and public transportation.
