Deliver treatments including wound care, medication management, and assessments . Coordinate care with physicians, therapists, and interdisciplinary teams. Educate patients and families on care plans, medications, and recovery goals . Maintain accurate, timely documentation in the EMR. What We’re Looking For Experience in home health, med-surg, TCU, rehab, or clinical nursing . Strong clinical judgment and ability to work independently.
