Vivia Health seeks an ACT Case Manager to deliver intensive, wraparound services to adults with severe mental illness in home and community settings across the Chicagoland area. This fully offsite, field-based role involves independent work, remote team check-ins, and collaboration with psychiatrists, nurses, and therapists to support clients in obtaining care, housing, benefits, and recovery services. Qualifications include IM+CANS certification, a BA/BS, and at least 1 year of relevant #J-18808-Ljbffr Vivia
