Position Status:Full timeShift:Variable Less than 12 hour shift (United States of America)Hours per week:40Job InformationExemption Status: Non-ExemptA Brief OverviewThe Substance Use Disorder (SUD) Coordinator provides direct substance use disorder services to patients across the hospital setting. This position is responsible for conducting comprehensive assessments of patients' substance use needs and identifying appropriate interventions. The SUD Coordinator completes substance use screenings and assessments, interviews patients and family members, and collaborates with interdisciplinary staff and community resources to evaluate the impact of substance use on the patient's physical, psychological, and social functioning. Based on assessment findings, the SUD Coordinator works collaboratively with patients, families, and the treatment team to develop and implement individualized intervention, placement, and discharge plans that support continuity of care and recovery.Education QualificationsMaster's Degree M.A., M.S. or related field RequiredExperience Qualifications2 years Experience in addiction or counseling services or related field. PreferredMedical and/or Social Work experience PreferredExperience in substance use disorder cases, assessments, and referrals to all levels of substance use disorder treatments. PreferredExperience in group work, case work, and community methodologies PreferredCustomer service experience PreferredSkills and AbilitiesExcellent communication and interpersonal skillsCase Note DocumentationKnowledge of organizational strategies and ability to apply those strategies in practice.Licenses and CertificationsLicensed Masters Social Worker -
