Sun River Health is currently seeking a full-time Social Care Network Care Manager to join our Health Home team in Suffolk County.Social Care Network Care Manager (SCN Care Manager) – TemporaryRole Details: The SCN Care Manager supports the implementation of a patient's integrated health care plan and works toward resolving patient needs and barriers. This role conducts screening and navigation services for patients eligible for the Social Care Network, with documentation completed in Unite Us and eCW. The focus is on providing individualized assistance to help patients navigate the health care system and community barriers, facilitating consistent and timely care across the continuum.Job Responsibilities / Competencies:Perform Health?Related Social Needs (HRSN) assessments and document findings in Unite Us and eCWProvide general navigation services and communicate program objectives to patientsAssist with scheduling appointments related to social drivers of health, including reminder and follow?up callsContact patients to support continuity of care and escalate cases requiring enhanced care managementCompile and distribute educational materials based on patient needsIdentify individual and community resources that support healthy lifestyles (e.g., transportation, exercise programs, community services)Maintain timely and accurate documentation of patient interactionsEngage approximately 25 new patients per weekPerform population management activities as assignedSpecial Skills & Knowledge:Demonstrated understanding of health care and social service systemsStrong ability to build effective relationships with patients and care teamsExcellent interpersonal communication and organizational skillsAbility to work independently and collaborativelyStrong time?management and problem?solving skillsPatience and capability to navigate complex systems of careProficient computer skills, with the ability to document concurrently in Unite Us and eCWMinimum Qualifications:Education: High School Diploma (required)Language: English (required); Spanish (preferred)Work Location: OnsiteStatus: TemporaryPay Range: $20/hour
