Social Worker

Schedule – Shift – Hours: Full-time – Day shift – 8:00am-4:30pm The Social Worker - Inpatient Psych acts as a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at the Hospital, providing leadership and psychosocial support necessary to address the individual's needs. Acting as a resource to patient/client, utilizes knowledge of community and health care resources to assist in addressing identified needs at any point of the continuum, pre‑admission to post‑discharge. Using timeliness and appropriateness of intervention, facilitates quality resource utilization (i.e., Length of stay and level of care).
