Community Care Case Manager: Field & Home Visits

Humana

Humana Inc. is seeking a Case Manager to support Wisconsin Family Care members, including frail elders and adults with disabilities. You will assess needs, develop an MCP, coordinate with a Field Care Nurse, and arrange non-medical supports while maintaining documentation. Travel up to 40% and a valid driver's license are required.
